Mercier Senior Center or attends trips sponsored by the Russell Mercier Senior
Center, must complete this form and return it to 14 Stonecroft Drive Hebron, CT

- "Help Us
Find Your House in an Emergency" (Hebron Volunteer Fire Department)
Form
The Hebron Volunteer Fire Department will make a
reflective sign with your house number on it, to aid them in finding your house
in an emergency. Please complete this form and return it to the Hebron Fire
Department, to order your sign.
- "How to
Alert 9-1-1 to Your Special Needs"
When a call is placed to the
9-1-1 alert center, the caller's home address appears on the screen.
Additionally, by filling out this form, the call center can be made aware of
your special needs such as visual, auditory, and mobility impairment. By
providing them with this information, they will be better able to assist you.

If you are looking to schedule
transportation with us, reading these guidelines will give you additional
information regarding when to call and schedule, the types of rides that we are
able to provide, and to whom we are able to provide them.
- Russell Mercier Senior Center Policies for Building Use During
Non-Operating Hours
The Town of Hebron allows the Russell
Mercier Senior Center to be used during non-operating hours, by non-profit and
town organizations that have a certificate of liability insurance, naming the
town as additional insured.
Those who fulfill the above criteria should
fill out the attached form and return it to the Senior Center for
approval.

part of Connecticut, NCAAA is a regional non-profit organization
that assists in the planning and organization of services
